documentation needs to be updated to reflect change to grid system

The documentation states that col-sm is "Collapsed to start, horizontal above breakpoints". As per https://github.com/twbs/bootstrap/commit/db45a60cc86190a060f0baf0b3961cbc690f3a65 this is no longer true. col-lg is required as well.
