True story from eons ago but just remembered today.

When I was doing an inpatient psychiatry rotation, there was a bipolar patient who called 911 from the public phone in the locked inpatient psychiatry ward. We heard about it from the nurses during morning rounds. The psychiatrist was nonplussed by such an unusual event-- calling 911 from within the hospital. The psychiatrist asked the patient why he would do such a thing. The patient said "I was feeling unwell. My heart was racing, I felt like I was going to throw up. I needed a doctor."

The psychiatrist said, "But we're your doctors." To which the patient said "No, I needed a real doctor."

He didn't say it to be a smart ass or to piss off the psychiatrist. He said it very matter of factly-- he felt unwell, he needed a doctor-- the other kind of doctors. Everyone suppressed a laugh. The psychiatrist smiled. Smart ass me wrote that the patient demonstrated good insight that day.
