I 2nd this I have been trying to find a doctor who would be able to fill out this form I keep getting the run around every time I bring this form up

Is that because you don't have a legitimate reason? Why would a doctor object if you have a legitimate reason?

Honestly I just feel like the doctors around my area are incompetent. They just keep giving me the run around. I provide all the proper documentation, and all I keep getting is this doctor is suppose to fill this out, then so on, I keep getting another type of doctor needs to fill out My girlfriend wants to get this document for herself but she has no valid excuse
