Genuinely, why does 191st constantly get filled with trash and used syringes, and 190th stay so clean (leaking groundwater aside). Is it the station managers? Is it that 190th is sloped? Is the 1 train just cursed? I seriously don't get it

Not even close IMO. Is true that there are more druggies in the East coast from DC to Boston now , specially post pandemic . But I donāt think is even close to anywhere in the west coast like LA, SF, Seattle , etc

Also in a better part of the neighborhood though nearby. Being West of Broadway makes a huge difference.
